How difficult is it?
That depends mostly on the photo and the number of colors. Real photos have shadows and highlights, so they usually have more and smaller sections than a simple design. A photo with a clear subject and a calm background stays relaxed to paint; a busy photo with 48 colors is a satisfying challenge.
